CG-4082 (CAC Required) CG-4082 documents all significant education, training, and qualifications not captured in your Employee Summary Sheet in CGBI. Guidance on what is authorized on a 4082 can be found in Chapter 4.c of the Performance, Training, and Education Manual (COMDTINST M1500.10c).The completed form should be submitted via the member's Personnel & Administration (P&A) office to the PSC Records Branch (MR) at HQS-SMB-CGPSC-BOPS-MR-SPOSupport@uscg.mil. OPM-4 will conduct career counseling for all officers who are NOT tour complete or do NOT anticipate transferring in AY21.
OPM-4 conducts on-line interactive counseling throughout the year via the Skype for Business application. Establishing a shared desktop is the most efficient means for an officer to review their record with OPM-4 and greatly enhances the counseling session.
